We have these live and running in 20 countries across Africa, the Middle East and South Asia. The problem with running a viable and functional system in the US is the current HIPAA, Hi-Tech, PPACA and ICD-10 compliance architecture and the Draconian AMA licensing requirements / expense issues with using the CPT coding. The systems work, and are mobile acceasible for payment, scheduling, referral, collaboration and tele-health. ObamaCare is standing in the way of the deployment of smart, patient centered, market driven health solutions.

There is a lot of 'discussion' about the relative benefits of public versus private cloud. Starting out, Public will most likely be cheaper, but the truth is that there is no one-size-fits-all solution and a private cloud can potentially be a cheaper - especially at scale.

Excellent points. AWS' marketing machine does have a tendancy to oversimplify their services a bit in their promotions. I always recommend clients to have a look and get a basic understanding but really leave the architecture design to professionals. Buen post!
