My client is based on the outskirts of Norwich so transport is preferred.

The role is to support adults with mental health issues in a newly refurbished hospital environment with excellent facilities.

You will be a key worker to the service user and actively follow the care plan and working closely with the qualified nurses in delivering care to all service user in the hospital and in the community

You will work as a key worker for designated patients to support and

guide the patient using care plans under the direct and indirect supervision of the Nurse-in-Charge,

utilising own initiative and personal strengths.

You will assist the service users with their daily routing including persoanl care and you will assist and plan education groups, creative groups etc, encouraging and helping

where necessary.

You will offer support to Service users when attending outside appointments, leisure activities

and other off site appointments and visits

Update care plans and daily logs is essential to good attention to detail is needed

You will encourage and assist the service users to actively look after themselves with personal grooming and light cleaning and domestic duties

You will participate in meetings with clinical staff and outside organisations.
