Looking for a Registered Nurse (RN) to esure the overall delivery of home health services provided to all patients in accordance with VNSNY policies, procedures and standards of practice.
As part of the nursing leadership team, supports the VNSNY mission, vision, values and strategic initiatives to ensure the highest level of patient-centered care, including quality and financial outcomes.
May provide post-orientation support to new hires during their transition to VNSNY and home care.
Works under general supervision.

Evaluates patients referred to VNSNY Home Care program to determine appropriateness of admission.
Promotes and fosters ongoing collaborative communication system with institutional personnel to establish coordinated home care.Manages and facilitates the safe and timely transfer of patients from a hospital or skilled nursing facility to the care ofthe Agency.
Works under general supervision.

Licensure: License and current registration to practice as a registered professional nurse in New York State required.

Education: Diploma in Nursing from a state approved program OR Associate's Degree in Nursing from a state approved program with at least one year experience as a full-time staff nurse in a medical/surgical nursing area OR Bachelor's Degree in Nursing from a approved program accredited by the National League of Nursing
- OR Bachelor's Degree from a non-approved program supplemented by content which can be equated to the approved program required.
Minimum of one year of clinical management, administration or discharge planning experience in a hospital setting required.
Minimum of two years of appropriate experience as a professional nurse/acute nurse, with at least one year in a certified home health agency (CHHA) or the equivalent preferred.
