Description

Provides case management for at-risk patients to ensure medication adherence by ensuring nurses answer questions and facilitate communication with physicians for patients experiencing difficulties with regularly taking medications; identifying and evaluating causes of patient non-compliance; providing education to patients on benefits of medication adherence; providing solutions to help maintain medication adherence; assisting in advising patients of the benefits of adhering to medication therapy; and sending clinical communication forms to physicians.
Manages environmental factors for project feasibility by assessing key opportunities; determining the strategic approach; identifying the current state of the business; conducting gap analyses; collaborating with cross-functional teams and stakeholders to assess needs and success measurements; and developing cross-functional assessments and business cases for action.
Manages the development and the execution of small to medium projects or programs by developing high-level plans to achieve strategic goals and business objectives; developing and adjusting timelines, tasks, and analysis of resource allocation; leading cross-functional teams; prioritizing and managing work streams; monitoring progress versus plan; prioritizing and ensuring resolution of critical issues; providing informational presentations; overseeing change management; managing stakeholder expectations; and developing implementation materials.
Reviews and monitors completed new patient evaluations by verifying that patients are contacted for new patient evaluation and that drug and disease information is collected; ensuring that patients are advised of side effects, dosage, and storage of medications; and ensuring patients are taught how to administer medication.
Leads the continuous improvement of tools and strategies by researching industry trends; benchmarking with industry leaders to determine innovative solutions; comparing industry best practices and solutions to Walmart strategies and capabilities; determining business impact of new solutions; partnering with senior leadership to ensure alignment with strategic direction; making recommendations to leadership regarding changes and updates to the strategic direction; and integrating innovative solutions into strategies for Walmart.
Monitors patients to ensure adherence, compliance, and possible side effects by ensuring nurses answer questions regarding possible side effects; ensuring nurses facilitate communication with physicians for patients experiencing difficulties with regularly taking medications; identifying and evaluating reasons for non-compliance; providing education to patients on benefits of adherence; providing solutions to help maintain compliance; assisting in advising patients of the benefits of adhering to medication therapy; and sending clinical communication forms to physicians.
