Minimum Annual Starting Salary Pay Grade J : $37,138 Minimum Annual Starting Salary Pay Grade K : $40,852

Minimum Annual Starting Salary Pay Grade L : $43,937 General Nature of Work
Provides care coordination and case management services to children who have complicated medical conditions. Assesses the patient’s condition, develops plans of care, educates patient and the family, and refers patient to agencies that can assist in meeting health and /or social needs. Documents pertinent observations and responses to treatment. Facilitates physician/patient communication & treatment via Telehealth. The position requires extensive travel throughout the 14 county Health District to include home visits. Conducts presentations at schools, churches, organizations, etc. May serve on community committees. Must maintain CPR certification. The position may require working some evenings and weekends during special events such as health fairs and responding to disasters. May be temporarily assigned to assist in other counties in the district. Maintains HIPAA standards related to Privacy and Security and provides exceptional customer service. Minimum Qualifications
Pay grade J: Associate's degree/diploma in Nursing from an accredited college AND Current Georgia Registered Professional Nurse license.
Pay grade K: Bachelor's degree in Nursing from an accredited college or university AND Current Georgia license to practice as a Registered Professional Nurse OR Associate's degree/diploma in Nursing from an accredited college AND One year of PHN experience AND Current Georgia Registered Professional Nurse license.
Pay grade L: Bachelor's degree in nursing from an accredited college or university AND One year of PHN experience AND current Georgia license to practice as a Registered Professional Nurse OR Associate's degree in nursing from an accredited college or university AND Two years of PHN experience AND current Georgia Registered Professional Nurse license.

•Experience in providing case management services
•Experience performing physical exams on newborns, toddlers, children and adolescents
•Baccalaureate level health assessment course
•Public Health experience

Qualifications
