Position descriptionThe Care Transitions Program (CTP) is offered through Employee and Community Health (ECH) for patients greater than age 60 who are transitioning from the inpatient to the outpatient setting. This program seeks to assist frail elders during this critical point of transition, improving health outcomes and decreasing 30-day readmission rates.
The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected. (012560)
QualificationsGraduate of a baccalaureate Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience.
Additional qualificationsApplicants to this position must have a minimum of three years current adult-care experience in one of the following areas: ambulatory (preferred) or acute care. BSN required. RN Coordinators hired into this role may provide cross-coverage within Employee Community Health areas as needed. Work location and FTE’s are subject to change based on the need. Applicants must demonstrate excellent communication skills (verbal and written). Experience working in a care-team environment is preferred. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making.

License or certificationMust obtain prior to start date:
Current RN license by applicable state requirements.
Arizona and Florida: Maintains Basic Life Support (BLS) competency.
Midwest: Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

American Heart Association (Title: BLS for Healthcare Providers)
Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)

Compensation DetailEducation, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $32.72 per hour.
